Clinical audit on the use of continuous vancomycin in patients admitted to an ICU of a trauma referral hospital

Abstract

Vancomycin is an antibiotic widely used in hospitals to treat serious infections, but there is controversy regarding its dosage and monitoring. The objective of this study is to carry out a clinical audit on the use of continuous vancomycin in patients admitted to the Intensive Care Unit of a state public hospital for trauma, through a retrospective, sectional and descriptive analysis, with a demonstration of convenience that encompasses all patients. adults continuously using vancomycin from January to February 2021, using data from medical records, the Pentaho ® system, SIGH®, InfoLab® and SAME. In total, 87 patients admitted to this adult ICU will receive continuous use of vancomycin, of which 57 meet the inclusion criteria. The average time of use was 11 days and the main reason for use was to treat meningitis. The average time required to achieve therapeutic success according to vancokinemia was 3.64 days and all patients received the loading dose, with 54% agreeing with the purpose of UTI. Two patients who used continuous vancomycin 49% had a dose adjustment according to standardization. 513 vancokinemia tests were performed and 646 clearance results were calculated. There was a 6% growth in Gram-positive microorganisms. This study demonstrates the importance of having a well-defined protocol to carry out adequate monitoring of patients on continuous use of vancomycin, aiming for effective therapy and seeking to minimize its adverse effects.
