Abstract

The rational use of medicines depends on the availability of reliable, up-to-date, and evidence-based technical information, especially in the context of public health within the Brazilian Unified Health System (SUS), where access to clinical decision-support tools is limited. This technical note proposes a Pharmacotherapeutic Guide for the public health network of the municipality of São Paulo, based on the Municipal Essential Medicines List 2 (REMUME), aiming to support multiprofessional clinical practice, strengthen patient safety, and promote the rational use of medicines. The proposal was based on a documentary analysis 
of a descriptive and exploratory nature, grounded in secondary sources, including a critical review of scientific literature and regulatory documents, as well as the collection and systematization of technical and pharmacological information on REMUME medicines
obtained from evidence-based clinical databases such as UpToDate®, DynaMedex® with Neofax® integration, and the Electronic Drug Information System of the Brazilian Health Regulatory Agency (ANVISA). Medicines were selected from REMUME, excluding those belonging to the strategic component of pharmaceutical services, magistral formulations, and off-label indications. The analysis revealed gaps in access to standardized pharmacotherapeutic information and weaknesses in pharmaceutical services resulting from the absence of guiding instruments for clinical practice. As a result, a standardized model of a Pharmacotherapeutic Guide is proposed, applicable to multiprofessional practice, with the
potential to improve pharmaceutical services, reduce therapeutic discrepancies, and strengthen the role of the clinical pharmacist within the municipality of São  Paulo's public health network.
